Indiana PathWays for Aging Medicaid Waiver: A Guide for Fort Wayne Families
As of July 2024, Indiana restructured its Medicaid waiver programs. The PathWays for Aging Waiver now serves individuals 60 and older who meet nursing facility level of care requirements. This guide explains how the waiver works and how it may help families in Fort Wayne access assisted living.
What Is the PathWays for Aging Waiver?
The PathWays for Aging Waiver is Indiana's Medicaid Home and Community-Based Services (HCBS) program for adults 60 and older. It allows eligible individuals to receive care in community settings, including assisted living facilities, as an alternative to nursing facility placement.
Eligibility Requirements
To qualify, an individual must be 60 years of age or older, meet nursing facility level of care criteria, and meet financial eligibility requirements that include both income and asset limits set annually by the state. Because these thresholds change each year and vary based on household composition, we encourage families to consult with a Medicaid planning professional or contact Indiana Family and Social Services Administration (FSSA) for the current figures that apply to their situation.
How It Works
Once approved, individuals are assigned to a Managed Care Entity (Anthem, Humana, or UnitedHealthcare) for care coordination. The MCE works with the individual and their care team to develop a person-centered service plan that may include assisted living services, personal care, and other supports.
